Access To Medical Records
Patients have the right to access their own medical records by arrangement with the practice manager.
Change of Address
Let us know as soon as you complete your move or change your telephone number. Please note, as per Government guidelines, the practice operates within a boundary based mainly on the postcodes BD17 and BD18. If your new address falls outside these boundaries it will, unfortunately, mean you will have to register with another General Practice nearer to you new home. If you are unsure whether your house falls within or outside the boundary please enquire via reception.
Complaints and Compliments
Mrs Bates, the practice manager, is happy to receive your comments about the services at Windhill Green and
Cliffe Avenue surgeries.
Should you have a complaint, we would like to hear from you straight away so that we may offer you a satisfactory explanation. If you remain dissatisfied, you may send a letter detailing the nature of your complaint which will be dealt with according to our Practice Complaints Procedure (copies held at reception or click here). If you do not feel your complaint has been dealt with satisfactorily by the practice, contact the Patient Advisory Liaison Officer at:

These are available at reception. If you have any queries or problems regarding these, please discuss them with your doctor.
Expert Patient
The Primary Care Trust (local health trust) currently runs sessions for patients wishing to learn how to cope more effectively with chronic conditions and help others care. If you are interested in becoming more involved in the development of services please contact the assistant practice manager, Mrs E Bates on 01274 595951.
Freedom of Information - Publication Scheme
The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the 'classes' of information the practice intends to routinely make available.

Home Visits
If you are too ill to attend surgery and require a home visit please ring before 10.30am. The receptionist may ask for some details of your problem to help the doctors decide the order of surgery.
Medical Research Council
The practice has been involved in projects for the MRC since the 1970s and continues to be involved in current research. If you are to be included in any of the research you will be contacted beforehand for your consent prior to any information being used in any research project. If you do not wish your records to be used please speak to your doctor or a receptionist. Your right to refuse will not affect your ongoing care within the practice.
Outpatient Clinics - The Emerald Suite
A counsellor is available weekly (by appointment, through your doctor) at Windhill. A chiropodist holds a surgery weekly - those under 70 years of age need to be referred by their doctor. A
Warfarin clinic is held fortnightly and an audiology clinic is held weekly.

If your GP decides to refer you to a consultant for further treatment, you will be offered a choice of where you can be seen. You will be directed to a member of staff who will be able to assist and guide you through the choices available to you and provide you with information about each of the hospitals. You will be given a unique booking reference number and information of how to book your appointment directly with the hospital. Or, if you prefer, your GP can still make the choice for you and recommend a hospital for your treatment.
Patient Involvement Forum
This forum is held at Windhill Green Medical Centre at approximately three-monthly intervals and is well publicised within the practice as well as in the local press. If you are a registered patient and would like to hear more about developments within the practice and hear our guest speakers on current topics relevant to healthcare and local issues, you would be very welcome. For the date of the next meeting refer to the notice board in reception or contact Mrs N Brook on 01274 595951.
Patient Satisfaction
The Practice conducts an approved patient survey annually and reflects on the outcome. Appropriate changes are considered and implemented to improve the patient experience when visiting the practice.

We hold daily clinics for ultrasound and weekly clinics for eye assessment.
Sickness Certificates
The standard sick noted is not issued for the first seven days of illness (you will need an SC1 from your employer or an SC2 if unemployed, available at the surgery). If you require a sick note from your doctor for the first seven days, a fee will be payable.

Consultants
Visiting consultants in dermatology, ophthalmology, ENT and neurology as well as a psychiatrist currently hold clinics. A consultant surgeon visits regularly to perform vasectomies. Referrals are made by your doctor.
